HEA 5005 - Management of the Critically Ill and Their Families – Multisystem Alterations
Course Description
Course 6 requires all learners to participate in both the shock and trauma sessions, as well as review all of the required self-study modules. The learner will be introduced to the critical care and emergency care of patients with multisystem dysfunction including shock and traumatic injury. Self-study will be required for procedural sedation, burn trauma, musculoskeletal, obstetrical, mental health, sexual assault/intimate/partner violence, and toxicological emergencies through online modules. Note: The course 6 exam will have questions on the self-study sections.
The ACLS component of Course 6 is delivered through self-directed learning and on-site training on days 34-36. The ACLS manual may be obtained from your site coordinator and there are 3 documents for your reference posted on D2L.
Learner Outcomes
Shock
- Define shock
- Identify the 4 stages of shock, etiology, and the relevance to patient assessment
- Define the types of shock
- Discuss risk factors, early recognition, assessment, and interventions/ treatment for shock
- Examine the shock cycle
- Review goal directed therapy for shock with focus on sepsis as published by the Surviving Sepsis Campaign (article for reference on D2L)
- Discuss case studies representing the different classifications of shock, including clinical presentation, diagnostic tests, medical management, and nursing care.
- Apply material from Course 1-6 NBCCNP
Trauma Tutorial
- Discuss the purpose and goals of the Trauma NB
- Understand the purpose/rationale for restriction of cervical spine motion to manage and maintain in patients presenting with multiple trauma
- Identify the importance of mechanisms of injury often associated with specific injuries of the trauma patient.
- Demonstrate appropriate technique in the application of a rigid cervical collar, pelvic binder and tourniquet.
- Demonstrate appropriate technique in log rolling the patient
- Identify the components of a primary and secondary trauma assessment
- Relate the importance of using a systematic approach in the management of the trauma patient
- Recognize life-threatening injuries, select appropriate interventions and evaluate the patient’s response.
- Discuss the concept of trauma team and the some of the roles and responsibilities of team members.
Each student will participate in a primary and secondary assessment of a trauma patient. They will be expected to demonstrate the application of appropriate trauma equipment on a trauma patient using the scenarios found on D2L. Students may work as a team during the practice and be expected to perform individually during the testing. A trauma evaluation will follow.
Trauma Evaluation
Each student will meet the criteria for successful completions of a Trauma Assessment (primary and secondary) as demonstrated by the successful independent completion of a trauma case scenario evaluated by onsite instructor.
Notes
Course Equipment and Resource Requirements
1. Shock: View online modules, Articles, Case Study (D2L), attend and participate in all instructor led online sessions. There are some articles for reference on D2L.
2. Participation: Marks are allocated for online class participation and case study analysis.
3. Video Assignment: There is a video of a scenario on D2L. It is listed as video assignment. To complete this assignment: Watch the scenario, there are at least 5 mistakes/ neglected actions it. For the assignment: list the 5 mistakes/neglects and give rationale for why it was a mistake or what should have been done differently. Also answer the questions accompanying the video assignment. You can use the shock and trauma lectures and materials on D2L and the online modules as references. Write this up and submit to Melanie.Didham@HorizonNB.ca by midnight of the due date. It does not have to be APA format, just clear and in a Word document please. This assignment counts as 15% of your course 6 mark.

Trauma Assessment Demonstration, Application of Cervical Collar, Application of Pelvic Binder, Application of Tourniquet and Trauma Assessment Evaluation
Site instructors will have available at each site a simulation station with the following items:
- Manikin
- Scoop Stretcher, rigid cervical collar, Pelvic Binder, Tourniquet, Airway and IV props, stethoscope
- Practice scenarios available on D2L
- Test scenario available on D2L for instructors
The Trauma delivery method includes student review of online material, videoconference, simulated scenario driven practice and demonstration of restriction of cervical spine motion/stabilization and primary and secondary assessment of the trauma patient lead by an instructor at each site.
Exam: Online computer exam includes Trauma/ Shock and specific self-study sections
